I have been searching, and have yet to see this type of error, or at the very least anyone having the same issue.
Ubuntu 10.0.4 (Karmic) installed syscp using apt-get install syscp, no modifications needed for the installer and setup to work. Everything is working, add users, domains, tickets, etc..
However, when a user attempts to create a database, the fields that are shown are Description, and password. (missing database name field?)
I have tried the users password, and the admin/root password for mysql, however this is the error I am receiving.

I know this fix is something quick and easy, but I just a point in the right direction as to what I am missing.
